How to do a name change in Harris County, Texas after divorce is finalized?

My friend has finalized her divorce in Harris County, but she didn't change her name at the time. She now wants to change her name but is not sure how to do it. She does not want to hire an attorney.

(The Harris County Clerk website has a form for a name change application, but that is only for use after the name change has been approved in the divorce decree)
